Visitors must fill in e-HDF before entering Sarawak

The e-Health Declaration Form (e-HDF) may be obtained from https://sarawakdisastermc.com/

During the Recovery Movement Control Order (RMCO), visitors at any point of entry are required to complete mandatory declaration form, the e-Health Declaration Form (e-HDF) before entering Sarawak.

The form is required to be filled by visitors travelling by air, including those travelling within Sarawak as well.

Failure to do so would result in a denial of entry to the State.

Under the management of the Sarawak Disaster Management Committee (SDMC), the data is completely confidential and will be used for the purpose of the prevention of Covid-19.

Here are the steps to steps guide to enter Sarawak:

  1. First, log into sdmc.sarawak.gov.my
  2. Create and log in into your profile
  3. Fill in your travel details
  4. Complete and submit enterSarawak application
  5. Once approval is issued, an approval mail and link to a e-HDF will be send via email.
  6. The e-HDF must be filled within 12 hours before arrival to destination.
  7. Visitors must show QR code to relevant authorities when required.
  8. If the e-HDF expires before your entry to Sarawak, you will be redirected to a Health Officer for health screening.
